Borderview Rehab & Living Center

Borderview Rehabilitation & Living Center is a 108-bed facility located in Van Buren – the “Gateway Town” to the picturesque St. John Valley. Borderview offers a full continuum of services that can meet any long term care need. We know that everyone has different needs at different times. In order to best meet your needs individually, we offer four different options of care:

Skilled Nursing & Rehabilitation Service offers a wide range of services and care with the ultimate goal of regaining independence. Our nursing staff specializes in IV therapy, wound care, pain management and patient education. Our therapy department consists of Physical, Occupational and Speech Therapy and provides services up to six days a week.
Nursing Care which offers an extensive level of care to maintain independence and meet our residents’ needs, with the goal of having our residents consider Borderview their home.
Sub-Nursing Care is offered to residents who require some assistance with activities of daily living, yet are not in need of a full range of medical services offered by traditional Nursing Care.
Residential Care offers residents the ability to live their lives as they choose, with 24-hour supervision, activities and individualized care planning.

Borderview is committed to service, and we encourage our residents to have choice and independence. To support our residents in creating a home like environment, we offer cable television in each room, internet access and private phone lines in most rooms. We also strive to make families a vital part of our residents’ lives and their care. We believe that keeping families informed and involved is one of the surest ways to make life enjoyable, both for our residents and for our staff. We support this philosophy through arranging family meetings and care plan meetings, and by giving priority to following up on your questions and feedback. Many residents refer to our caregivers as new members of their “extended family”. We hope you will do the same.
