THE FOREGOING ARE THE ONLY OFFICIAL MEANS FOR FILING COMPLAINTS/CONCERNS WITH THE VILLAGE AND WILL ENABLE THE VILLAGE TO RECORD PROPERLY SAID COMPLAINTS/CONCERNS AND TAKE RELATED FOLLOW-UP ACTIONS AS NECESSARY.
